IMPORTANT: Because your appointment time is reserved for you only, we ask you to comply with our office policies. To address many no-shows, effective 1/1/2023, we reserve the right to cancel any appointment that is not in compliance with the policies listed here. |

Patient Forms |
It is office policy that all New Patients have to complete new patient forms 3 days before the appointment or within 8 hours of booking one if your appointment is coming up in less than 3 days.

Insurance Information |
Thank you for providing your insurance information during booking. Sometimes, that information is incorrect or we may require additional information to verify your benefits. In this case, we ask that you get back to us as soon as possible. We reserve the right to cancel any New Patient appointment without proper coverage information. |
Confirming your first appointment |
We require that all patients confirm their appointments promptly. The 1st visit must be confirmed 7 days in advance or at the time of booking if the appointment is within the next 7 days. |
Appointment confirmation policy for returning patients |
We reserve the right to cancel any appointment that is not confirmed at least 72 hours before the appointment time. |
Grace Period |
Our grace period is 10 minutes.
We understand that different circumstances can affect your arrival at the office. However, to ensure that all the patients are seen on time we cannot extend the courtesy period. |
X-rays Policy |
For the doctors to perform an exam, consultation or any dental treatment, dental x-rays are required at the time of the visit.
If you are not eligible for x-rays due to insurance frequency limitations, we will ask you to provide the x-rays from another office or pay out of pocket for new x-rays. We will not be able to see you unless we have the x-rays on file at the time of the visit. |
Cancelation Policy |
Because your appointment time is reserved only for you, 24 hours is required in the event you need to cancel your appointment.
Cancellations made with less than 24 hours notice will result in a cancellation fee. Patients who do not show up to a scheduled appointment without a call to cancel will be considered as “no show.” No show appointments are also subject to the cancellation fee.

Can I cancel my appointment? |
Yes! As long as you let us know 24 hours in advance prior to your appointment, we’ll cancel your appointment without any cancellation fee.
If you cancel your appointment less than 24 hours from the appointment time, you’ll be charged a Cancelation Fee. |
Can I reschedule my appointment? |
Yes, you can, but the same rule applies for rescheduling. You must do so 24 hours prior to the scheduled appointment. |
